Silver Years Group is a PBOT-licensed transportation provider serving clients who are eligible through the Oregon Health Plan (OHP).

We provide non-emergency medical transportation (NEMT) exclusively for authorized OHP members in coordination with approved transportation networks.

Our drivers and caregivers are professionally trained, CPR-certified, and experienced in supporting seniors and adults with special needs.
Our team includes Russian- and Ukrainian-speaking staff, ensuring clear communication, comfort, and safety.

• Welcome to our Adult Daycare Facility, a compassionate and nurturing space designed to enhance the quality of life for older adults and provide respite for their caregivers. Our comprehensive range of services is tailored to meet the diverse needs of our participants, promoting social engagement, cognitive stimulation, and overall well-being. We operate during the daytime hours, Tuesday through Friday.
• Personalized Care Plans: Each participant receives an individualized day-program plan developed in collaboration with their family and any external healthcare providers involved in the participant’s care. This plan ensures that our services cater to specific preferences, health conditions, and interests, promoting a holistic approach to care.
• Social and Recreational Activities: Our facility offers a vibrant social environment where participants can engage in a variety of recreational activities. From arts and crafts to games and group exercises, there's always an opportunity for social interaction, fostering a sense of community and friendship.
• General Wellness Observation and Medication Reminders: Staff provide general wellness observation and communication with caregivers regarding participant well-being. Participants remain responsible for their own medications unless otherwise arranged with authorized caregivers or healthcare providers; staff may provide medication reminders and support for participant self-administration when appropriate.
• Nutritious Meals and Snacks: A balanced and nutritious diet is crucial for overall health. Our facility provides well-balanced meals and snacks, taking into consideration dietary restrictions and preferences. We prioritize healthy eating to support physical and mental well-being.
• Transportation Services: For those requiring assistance with transportation, we offer convenient options to and from our facility. This service ensures that participants can easily access our programs while easing the burden on caregivers.
• Wellness Activities: We offer wellness activities, movement activities, and cognitive engagement activities, including exercise and recreational programming. These activities are designed to support participants’ general well-being and social engagement in a day-program setting.
• Cognitive Engagement Activities: For participants experiencing memory challenges, we offer cognitive engagement activities, including memory exercises, reminiscence-style group activities, and recreational programming designed to support participants in a social day-program setting.
• At our Adult Daycare Facility, we are dedicated to creating a warm and inclusive environment that promotes dignity, independence, and a fulfilling quality of life for all participants.

Emotional wellness and social connection are an important part of our adult day program. Through group activities and a welcoming community environment, we help participants stay engaged, connected, and supported in their day-to-day experience.
